I really like Easy Forms, I use it with all my mail chimp subscriptions on my websites.

Form doesn't show up

Installed fine, but the sign-up form doesn't appear on my site unless I'm logged in. I tried clearing cache, turning off the CDN, even loading the site in a private window. The form only shows when I'm logged in, even though I specifically checked that the option to prevent this was in effect.
